Yes, it is PT-196:
"PT-196, the last of the original contract boats, had a series of light grey stripes and a shark's face added to the original Measure 14, along with the nickname 'Elcopuss', upon completion. She reverted to a more normal Measure 31/5P in service."

So I'm guessing those smoke stack looking things are turret covers for the twin .50s, with them stowed upward?

And Italeri now has the early PT-103 Class 80' Elco boat kit out as PT-109 which has the torpedo tubes you need.
