Found these pics on an IDF modeling site:
According to the info provided, the caption reads: "a pair of electrically fired rocket pods were installed on a Patton tank of the 52nd battalion of the 252nd division. The rockets' firing mechanism was controlled by the tank commander. Live fire tests were performed at the Ruefa dam, to the west of Nitzana"
I am pretty sure this is a Magach 5 (again per the other posts at the site) But really unique either way!
